SECTION 1: MEDICAL DISCLOSURE & RESPONSIBILITY - I affirm I am physically and mentally fit to participate in training at Good2Go Mixed Martial Arts. I have disclosed all medical conditions that could affect my safety. I understand it is recommended that I receive a physical examination and physician’s clearance before participating in any martial arts or combat sports programs. I assume responsibility for participation regardless of whether I’ve obtained medical clearance. I understand that instructors are not medical professionals and cannot diagnose or treat injuries. I accept sole responsibility for monitoring my health and fitness throughout my participation.

SECTION 2: ASSUMPTION OF RISK - I acknowledge that martial arts involve inherent risks, including but not limited to: fractures, strains, neurological injury, cardiac episodes, and exposure to infectious diseases (e.g., COVID-19, MRSA, etc.). These risks include those caused by the actions, inactions, or negligence of others. I voluntarily accept and assume all risks. I have read and understand the Organization’s Risk Management Protocol, including facility rules, training safety procedures, and instructor conduct expectations.

SECTION 3: RELEASE AND WAIVER OF LIABILITY - In consideration of participation, I hereby RELEASE, WAIVE, and HOLD HARMLESS Good2Go Mixed Martial Arts, its officers, directors, employees, coaches, agents, volunteers, and instructors (the “Releasees”) from any and all claims for injury, illness, death, loss, or damage to person or property, EVEN IF ARISING FROM NEGLIGENCE of the Releasees. I agree to INDEMNIFY and DEFEND the Releasees from claims brought against them related to my participation, except where due to their gross negligence or intentional misconduct.

SECTION 4: INSURANCE & LIMITATIONS - I understand that Good2Go Mixed Martial Arts may not carry insurance that covers all injuries, illnesses, or damages sustained during activities. I agree that I am financially responsible for any medical expenses not covered by personal insurance. D&O insurance is maintained to protect leadership in accordance with our nonprofit obligations.

SECTION 5: MINORS If the Member is under 18: I am the parent/legal guardian and have explained the risks and responsibilities outlined above to my child. I agree to complete a follow-up liability release when the minor turns 18.

SECTION 6: USE OF IMAGE & MEDIA - I consent to the use of photos, video, or audio taken during participation for promotional and training purposes by the Organization.

SECTION 7: ENTIRE AGREEMENT This agreement reflects the entire understanding between the parties. Any modifications must be in writing and signed by both the Member (or guardian) and an authorized representative of the Organization.

By signing below, I affirm that I have read this agreement in full, understand its contents, and voluntarily agree to all terms. I am aware that by signing this agreement, I am waiving certain legal rights, including the right to sue for injuries or damages, as permitted by law.

Warranties-There are no warranties either express or implied in this Agreement that are not expressly contained in this Agreement.

This waiver applies to all current and future participation unless revoked in writing.

Applicable Law-This Agreement shall be governed by the laws of the State of Virginia.

In the event of a legal dispute between myself and Good2Go Mixed Martial Arts, each party will be responsible for their own legal fees and court costs. However, if a court determines that a claim was made without merit or primarily to cause harm or disruption, I understand that I may be required to cover the Organization’s legal expenses.

Any legal action shall be filed in Shenandoah County, Virginia.
